YOUTH REGISTRATION FORMS

  • For youth teams (ages 14–18) registering for tournament participation. Team managers or coaches should complete this form on behalf of their team.

    Once a team is registered, each player must complete the Individual Player Registration form before participating.

If you are already connected to Portland Plays Together access programming, please reach out to your community organizer, coach, school representative, or partner organization for registration instructions before completing a form.

If you are not participating through a community partner, please use the standard Youth Team, Youth Free Agent, Adult Team, or Adult Free Agent registration forms instead.
