Player Registration Information
This form must be completed by every player participating on a registered youth team.
Individual player registration allows Portland Plays Together to collect required participant information, emergency contact details, apparel sizing, and participation acknowledgements.
After registration
• Your information will be associated with your team's registration.
• Tournament updates and event information will be shared through your team representative.
• All required registration information must be completed prior to tournament participation.
Please have your team name, emergency contact information, and apparel sizing available before beginning registration.
The information collected helps us organize the event efficiently, maintain participant records, and improve future programming opportunities. All personal and sensitive information provided through this form will be kept private, secure, and confidential. Information will only be used for tournament administration, communication purposes, and internal reporting, and will never be shared with unauthorized third parties.
