Before You Register
Team Registration Information
This form should be completed by a team captain, manager, coach, or designated team representative.
Team registration reserves your team's place in the Portland Plays Together Adult Tournament and serves as the primary point of contact for tournament communications.
After registration:
• You will receive a confirmation email from Portland Plays Together.
• Each player on your roster will be required to complete the Adult Player Registration & Waiver form before participating.
• Tournament schedules, event information, and team updates will be shared with the contact information provided below.
• Teams are responsible for ensuring all players complete their required registration prior to tournament day.
Team Entry: $350
Roster Size: Up to 10 Players
Minimum Matches Guaranteed: 3
The information collected helps us organize the event efficiently, maintain participant records, and improve future programming opportunities. All personal and sensitive information provided through this form will be kept private, secure, and confidential. Information will only be used for tournament administration, communication purposes, and internal reporting, and will never be shared with unauthorized third parties.
